Delegated utilization management and benefit managers

2 min read

For several service categories, the health plan does not perform utilization review itself. It delegates to a specialist vendor with its own criteria, its own submission channel and its own timelines. Requests sent to the plan instead are not reviewed; they are redirected, and the redirection costs days.

#Commonly delegated categories

CategoryTypically covers
Advanced imagingCT, MRI, PET, nuclear cardiology. The longest established delegated category.
CardiologyDiagnostic catheterisation, echocardiography, advanced cardiac imaging.
MusculoskeletalSpine surgery, joint replacement, interventional pain procedures.
OncologyChemotherapy regimens and supportive care, often via pathway programmes.
Radiation oncologyTreatment planning and delivery.
Genetic and molecular testingA fast-growing delegated category.
SleepStudies and therapy devices.

The vendors operating in this space are a small set, and a practice will meet the same names across many plans. Their criteria are frequently published and, as with plan medical policy, reading them once for your common procedures is the highest-yield preparation available.

The redirect is silent and slow
A request sent to the plan for a delegated service is often not rejected outright. It sits, then comes back as a direction to submit elsewhere, days later. Nothing was reviewed. Knowing in advance which categories each plan delegates, and to whom, removes a recurring multi-day loss for zero ongoing cost.

#What differs once you are there

Criteria
The vendor's own, not the plan's, and frequently stricter or more specific for the delegated category.
Channel
A separate portal, separate credentials, and sometimes a separate EDI arrangement.
Timelines
Set by the vendor's contract with the plan. Regulatory timelines still reach the plan, and the plan remains accountable, but the practical clock is the vendor's queue.
Appeals
The first level is usually with the vendor. The route back to the plan, and onward to external review, varies and is worth establishing before you need it.
Peer-to-peer
Conducted with the vendor's physician reviewer, who is generally a specialist in the delegated area.

#What to record

One table, maintained per payer: which categories are delegated, to which vendor, through which channel. It changes when contracts change, which is infrequent, and it removes an entire class of avoidable delay. Most practices reconstruct this knowledge informally in individual people, which works until those people are unavailable.
